What is a Stress Fracture of the Knee?

A stress fracture, or subchondral insufficiency fracture, of the knee occurs when repetitive and excessive stress is applied to the knee. The medial side of the knee is most commonly affected. Patients with low bone mineral density and post-menopausal females are at higher risk.

Symptoms of a Stress Fracture of the Knee

Patients with a stress fracture of the knee usually experience severe, atraumatic knee pain, stiffness and limping that is sudden in onset. Pain tends to improve with rest and worsen with weight-bearing.

Diagnosis of a Stress Fracture of the Knee

Knee stress fractures are diagnosed with a combination of patient history, physical examination and imaging studies. When X-rays are benign and symptoms are severe, an MRI is commonly obtained to solidify the diagnosis.

Treatments for a Stress Fracture of the Knee

Stress fractures of the knee are initially managed conservatively. This entails a period of rest, anti-inflammatory medicines, a knee brace for compression and support, protected weight-bearing using a walking aid, and physical therapy. Injections are not recommended in the setting of a stress fracture. Surgery is considered when symptoms fail to improve with a dedicated course of non-invasive measures.
